Primary Care

West Yavapai Guidance Clinic operates the “West Yavapai Primary Care” Practice, providing medical services for adults who are enrolled at the Clinic. Primary Care includes diagnosis and management of chronic conditions, or those of an acute but non-emergent nature.

Ferenc (Frank) Nagy, MD serves patients Monday, 8:00 am till 11:00 am; Tuesday and Thursday, 8:00 am till 12:00 pm, and Wednesday, 9:30 am till 2:00 pm.  While Amy Thein, NP serves patients Monday – Thursday, from 8:30 am till 11:00 am.

The vision is to provide closely coordinated care between primary and psychiatric health care providers, to help patients achieve improved health care outcomes, to intervene early related to chronic conditions and to improve patients’ quality of life.

Who is eligible?

  • Enrolled clients of West Yavapai Guidance Clinic who are on United Health or U of A Health Plans
  • Enrolled clients who are dual-eligible with United or U of A and have Medicare
  • Enrolled clients who have traditional Medicare coverage, Medicare with any supplemental plan, and selected Medicare replacement plans. Specifically, the insurance plans with which we are contracted (as of 1/13/15) are:
    • MEDICARE REPLACEMENT (ADVANTAGE) PLANS –
      • United Health Care
      • Desert Canyon (Arcadian)
      • U of A Physicians
      • Abrazo
    • MEDICARE SUPPLEMENTAL (SECONDARY) –
      • AARP

If you are an eligible adult client of West Yavapai Guidance Clinic and have been considering changing your primary care provider, or you are not currently seeing a primary care provider, please contact your AHCCCS health plan. The health plan can help make this change happen.

You can also contact West Yavapai Primary Care directly, 583-6411, ext. 3805, Mon-Fri between 8:00 am and 4:00 pm. And, feel free to discuss this with your behavioral health service coordinator, or your psychiatrist or psychiatric nurse practitioner.

Meet your West Yavapai Primary Care providers:

Ferenc “Frank” Nagy, MD is already a familiar name to many in the Prescott area, having practiced primary care medicine at Valle Del Sol and Prescott Valley Urgent Care during recent years. His extensive education and training includes graduation from McGill University in Montreal, Canada, followed by training at Montreal General Hospital, McGill University and St. Paul’s Hospital in Vancouver, Canada . He is also a Certified Geriatric Medical Director (AMDA), and a Retired U.S. Army Colonel, Medical Corps, Desert Storm Vet.

Amy Thein, MSN, FNP-C, CDE, PMHNP-BC is Board Certified with the State of Arizona as a Family Nurse Practitioner. She recently worked with Veterans through the Northern Arizona Veterans Healthcare System, and she served both adult and pediatric patients through Prescott Valley Family Medicine. Ms. Thein earned her Family Nurse Practitioner credential through the University of Arizona.  She has the unique qualifications to also serve the psychiatric needs of WYGC patients, having earned her Psychiatric Mental Health Nurse Practitioner also at the U of A.

Are you seeing Dr. Nagy for primary care, and you have a mental health condition that is referred to as “serious mental illness,” also called “SMI”? If so, you may be eligible for TLC!  You may also be eligible if you are a WYGC client and have a primary care provider outside of our agency.  TLC stands for:  Total Linked Care

Do you have one or more of these, or are you at risk for one or more of these?

-diabetes

-heart disease

-high cholesterol

-breathing problems such as COPD or asthma

If so, call us or ask your case manager how you can get started with your own TLC wellness coach and personalized health plan.

Thanks to an Integration grant from the federal agency SAMHSA (Substance Abuse Mental Health Services Administration), West Yavapai Primary Care can provide this exciting new program.
